You "expose" your IP address just by surfing to the site, registering doesn't add any additional risk. Users do need to give an e-mail address, but this can be a webmail account created anonymously for exactly this purpose.
Why are you paranoid? Are you worried about surfing from work? Your spouse?, or something else?
There are many inexpensive VPN services available if you want to avoid IP adddress tracing, and coincidentally want to get around the "Great Firewall of China" when you're in the PRC (which blocks sex141.com). One of the famous ones is
relakks (from Sweden). I use one of these when I'm in China, but also need it for certain hotels that just have weird internet setups.
