Have done it many times.  Very easy.

Unroll a condom, cut from base towards the top, don't cut all the way.  Leave the top in tact to use with possible toys/fingers.  Stay away from "endurance" condoms or anything with numbing... try to stay away from anything with spermacide as the taste is very unpleasent.  Lightly lubricated is ok.  Unlubricated is best.
