I watched some of the replays and you can see Hardy tap GSPs ass once and GSP looks up at the ref while easing up on Hardy, at which point Hardy escapes. But GSP was too nice though, he could have easily broken Hardy's arm either time.

The ref did not get in there quick enough because Carwin was got in 3-4 solid punches to the side/back of Mir's head after he was out cold. Even if Mir comes back, he'll never be the same again.
