Also for me condoms break because the girl has become dried out from too much thrusting and there is no gel on hand to 're-wet' the girl.

Yes, not holding the base of the condom has been the issue at times.  On occasion if the girl is on top and pumping away like in a crouching cowgirl (a la spider monkey) position then there is a chance it may slip off.
