Me, I stick with the eye contact, buy them a drink, and a dumb old, "you from _______?" The typical, "My name's _____" also works.  I guess the simple approaches make you come off as harmless, and girls like that initially in a bar type situation.

I'm slightly rusty as i've been in a relationship for 3 years. However something that used to work for me was as follows (more of a pickup technique than line). After scanning appropriate social setting spot a pair of girls. You'll know what i mean when I say there is usually always one fit one (good looking) and one ugly/ not so fit one. I'll then try and find a natural way to start talking to the ugly one, usually ask them a  genuine question "do you know what time this place closes tonight?" then lead onto friendly conversation. At the time not paying much attention to the pretty girl (who is used to a mans attention) though at the same time not being rude to her. After a while of moderate flirting/small talk find an excuse to go, e.g. i'm going to get a drink/ toilet. On your return single out the pretty girl who will by now be a lot more receptive to your attention. As you have hopefully come across as friendly, non-threatening and not too superficial by chatting with her ugly friend and you're now giving her the full attention she craves. You then pretty much take it from there. Another variation is to get a friend to pair off with the ugly girl and "keep her busy" if you will.
