Simple solution to that one: don't EVER UNDER ANY CIRUCUMSTANCES, WHATEVER THE TEMPATION, speak or even think the name of your partner during sex. ANY partner. Never.
Plenty of alternatives most girls actually prefer: "honey" "my love" "darling" "bitch" etc etc
that way you've got a chance that if you are asleep, or half asleep, or getting lost in a fantasy deeper than Inception, whatever comes out has a chance of being fail-safe.

honestly, think about it: the majority of the work done by counsellors and therapists relates to couples fighting over something, and the majority of what they fight about is sexual cheating. I believe few people go to see a counsellor BEFORE cheating, neither men nor women.
Basically two rules:
until it happens assume it will be the end of everything as you know it, and act accordingly. Partition. Build Chinese Walls. Constantly scrutinise your cover story. Maximise plausible deniability. Reflect on backup plans. And above all keep things as normal as you can.
WHEN it happens (and assume it will happen), do NOT assume it's the end of everything as you know it ...
what happens next is completely unpredictable, as it depends 100% on your unique individual circumstances, and further depends on what happened and how you were caught, and culture, and history, and her personality, and your personality, and, and, and ... It ALSO depends on what your own intentions are, and your skill in handling a tense situation.
If it's not worth the risk, don't do it.
and a corollary: if there's any less risky way to do what you need to do, choose the less risky way.
